35 Years Fighting Bacteria in Hospitals Taught Me to Look at Your Shower Differently

We have spent the better part of our careers in environments where bacteria can kill. Hospital bathrooms, shower facilities in care homes, the water systems of surgical units. In these settings, you don't get to ignore the science. The consequences of doing so are immediate and serious.

For 35 years, our work was about understanding where waterborne bacteria grow, how they spread, and how to stop them. We used that knowledge to protect some of the most vulnerable people in the country: patients with compromised immune systems, people recovering from surgery, the elderly in residential care.

What I didn't fully appreciate until relatively recently was that the same problem was quietly present in almost every home in the country, completely unaddressed.

The moment it clicked

The shift happened when I started looking properly at the published research on domestic shower heads. I was aware of the clinical picture, the risks in healthcare settings were well documented and we'd spent decades managing them. But the research on household showers had been building for years, and the findings were more striking than I expected.

A study published in the Proceedings of the National Academy of Sciences found that opportunistic pathogens in showerhead biofilms were present at concentrations over 100 times higher than in the tap water feeding those same showers. A follow up study covering households in both the United States and Europe confirmed that Mycobacterium species, bacteria we take very seriously in hospital settings, were near universally present in domestic shower heads.

In clinical environments, we treat this kind of contamination as a serious and urgent problem. In domestic settings, nobody was doing anything about it at all.

What hospitals know that households don't

In a hospital, a shower that might expose patients to waterborne bacteria gets attention. There are protocols, monitoring programmes and engineering controls. The shower head material matters. The flow rate matters. The temperature matters. The frequency with which the system is flushed and the biofilm managed matters.

None of this complexity needs to exist in your bathroom. But the underlying principle does: a shower head that sits warm and damp between uses will accumulate bacteria over time, and the bacteria it accumulates can reach the person using it via the aerosol produced when the shower runs.

What we built, and the compromises we refused to make

My co-founder Tim Jones started working on what would become the Fereno RefreshFlip because we knew the technology to address this problem in domestic homes didn't exist anywhere. We'd been applying it in clinical settings for years. The question was whether we could bring it into a domestic product without sacrificing what makes it actually work.

Some of the products that claim to address shower head bacteria use surface coatings or silver ion technology. These can have an initial effect, but they degrade quickly and never reach the interior biofilm where the bacteria actually live and multiply. The approach we use operates differently. The antibacterial action is built into the material and mechanism of the shower head itself, not applied as a surface treatment.

After 35 years of working in environments where this is taken seriously, I find it remarkable that domestic shower heads have gone largely unchanged in this regard for so long.
